OSHA Record Keeping

OSHA 300 log available

OSHA’s Log of Work Related Injuries and Illnesses (revised in 2004 to include a new occupational hearing loss column) is available on the agency’s website at http://www.osha.gov/recordkeeping/new-osha300form1-1-04.pdf

 

The new form has the date of revision (rev. 1/2004) located on the form next to the form number. The new 300A Summary Form that includes a hearing loss column is also available and should be posted in February 2005.

 

Employers may continue using workers’ compensation First Report of Injury forms in lieu of the OSHA Form 301 Injury and Illness Incident Report as long as the workers’ comp form includes all of the same information as the OSHA form.
