CBIA offers members premier life and disability insurance products through our partnership with The Hartford. This partnership enables us to provide more competitive pricing, compensation options and flexibility for you and your employees.
- Eligible groups must have a minimum of three enrolled employees at all times. If a group has less than three employees at renewal, coverage may be canceled.
- Life and AD&D are mandatory if you participate in CBIA Health Connections. For companies with 10 or more eligible employees, life and disability programs are also available as stand-alone products. Please refer to plan details for an overview of available plan designs, limitations, and requirements.
- For life or disability claims, please use the claim form/application found here.
- All life and disability claim forms/applications for benefits should be submitted to CBIA. CBIA will work with Hartford Life to set up the claim in their system. Once a claim determination has been made by Hartford Life and the claimant has been notified, all inquiries should be made directly to Hartford Life.
