CBIA Health Connections and The Hartford created this supplemental life insurance program to be available only through CBIA in conjunction with your group’s May 2011 renewal. In this program, each employee who currently has basic life insurance through CBIA Health Connections may elect up to $100,000 in additional coverage.

If your company already participates in this program, you can offer non-participating employees a chance to enroll for a flat $10,000.*

For employers who are NOT CURRENTLY enrolled in the supplemental life program.

For employers who are CURRENTLY enrolled in the supplemental life program.

• For groups that currently offer supplemental life, we are requiring use of a pre-filled enrollment form. This is due to the complexity of age determination which is based on rate guarantee and group effective date. Please create pre-filled enrollment forms here or request them through your agent.
