New Personal Health Application Required for The Hartford

A new Personal Health Application (PHA) for The Hartford products (Life, ADD, LTD, and STD) has been published to our Web site. This new PHA is required to administer your clients’ group benefits policy and for their employees to submit evidence of insurability. Evidence of insurability is required for late entrants and those requesting amounts over the Guarantee Issue limit.

Your clients must begin using this new PHA immediately.
The Hartford will no longer accept CBIA’s Family Health Statement
for evidence of insurability.

NOTE: When applicants enroll under normal circumstances and evidence of insurability is not required, this new PHA document should not be submitted and does not replace the current CBIA Health Connections Family Health Statement.

Your clients are being notified of this new form. They may complete an interactive version and download additional copies online.
