Vision One Discount Program

For existing CBIA/Aetna members only.

Get "Eye-Opening" Discounts

The Vision One discount program is available to Aetna members through Cole Vision at no additional cost. This program may help you and your family save on many eye care products, including eyeglasses and contact lenses, nonprescription sunglasses, contact lens solutions and accessories.

Now, Vision One also offers a discount on the laser vision correction procedure called Lasik (Laser In-Situ Keratomileusis).

If your health benefits plan does not cover eye exams, you can receive an exam at a discount through Vision One. Schedule an appointment with a Vision One optometrist and receive an eye exam for $34; a contact lens eye exam is $10 off the regular fee. Most Vision One locations have doctors of optometry practicing on the premises or at a location nearby.

It’s as easy as 1-2-3

The Vision One program is available at many optical centers nationwide - such as Sears, JCPenney, Target, participating Pearle Vision centers and others - as well as through selected independent optometrist and ophthalmologist offices.

  1. Call Toll-free for the location nearest you.
    Call 1-800-793-8616 to access the automated locator 24 hours a day, 7 days a week. Representatives are also available weekdays from 9 a.m. to 9.p.m. ET and Saturdays from 9 a.m. to 5 p.m. ET. Or go to the Aetna web site at www.aetnaushc.com and click on DocFind, then select Vision One.
  2. Schedule an eye exam.
    Regular eye exams are an important part of preventive care. If your health benefits plan covers eye exams, you should follow your plan’s guidelines and consult your provider directory to choose a participating optometrist or ophthalmologist to administer the eye exam. Your out-of-pocket expenses could be lower if you follow the plan requirements. Many Vision One providers may be part of Aetna’s independent provider network.
  3. Save on eyewear.
    When you visit a Vision One location, show your Aetna ID card, and any applicable services or merchandise you receive will be discounted right at the point of purchase. There are no claim forms to complete and no waiting for reimbursement.

Need to Replace Contact Lenses?

After you purchase your first pair of contact lenses at a Vision One or other eye care location, you can then take your prescription to Vision One for additional pairs. Vision One gives you two ways to purchase these replacement contact lenses. You may have your prescription refilled at a participating Vision One center. Or you can order replacement contact lenses through the mail, using the Vision One Contact Lens Replacement program. You will receive the brand-name lenses your doctor prescribed, but generally at a lower cost. For more information about the Vision One Contact Lens Replacement program, call 1-800-391 LENS (5367).

Interested in Lasik surgery?

Through Vision One, you and each member of your family can receive 15 percent off the provider’s usual charges for Lasik surgery - the laser vision correction procedure - through Cole’s National Lasik Network,. Included in the discounted price is patient education, an initial screening, the Lasik procedure and follow-up care. If you are found not to be a candidate for this procedure, you will not be charged for the initial consultation.

To find the closest surgeons*, call 1-800-422-6600 and speak to a Lasik customer service representative. Then, contact one of these providers to schedule your initial screening. When you go for your visit, show your Aetna ID card and indicate that you are part of the Cole Vision One discount program. If you are found to be a candidate and wish to have the procedure performed, just schedule a surgery date with the provider, Then, call Lasik Customer Service (at the number above) with the date of your surgery and pay a deposit not to exceed $300 per eye. At that time, you will receive an authorization number so that you can receive the appropriate discount. In addition, your surgeon will receive written confirmation verifying your discount and the amount of your deposit. Your discount and deposit will be deducted from the surgeon’s fee at the time of treatment.

The Program

Taking care of your vision is important of your overall health, and Aetna helps make it easy to obtain routine eye exams. You can visit a participating optometrist or ophthalmologist for a routine eye exam*, without a referral, according to the schedule below. A copy applies.

If you wear eyeglasses or prescription contact lenses:

If you need a vision exam other than a routine checkup, consult your participating primary care physician, who can refer you to the appropriate participating eye care provider for evaluation and treatment.

Allowance for Eyeglasses and Prescription Contact Lenses

Your plan may offer an allowance to help you with the purchase of eyeglasses or prescription contact lenses. See your plan documents to determine if an allowance applies.

There are two ways you can get an applicable allowance:

  1. Purchase your prescription eyewear at a Vision One location and your allowance will automatically be deducted from your bill at the point of purchase. You can also get access to discounts on your eyewear and nonprescription items such as a contact lens solutions through thousands1 of Vision One locations throughout the country. Call Vision One at 1-800-793-8616 for the location nearest you.
  2. Purchase your eyeglasses or prescription contact lenses at an eye care provider other than a Vision One location and mail the paid receipt to us.** You’ll be reimbursed up to the maximum amount specified in your plan documents. However, if you use this option, you won’t be able to get Vision One discounts for eyeglasses or prescription contact lenses.

Contact Lens Purchase and Replacement

After you purchase your first pair of prescription contact lenses locally, you can get additional pairs through the mail. And you can get any applicable allowance and Vision One discount, too. Call the Vision One Contact Lens Replacement Center at 1-800-391-LENS (5367).

You may also purchase prescription contact lens replacements from any eye provider, submit your receipt to Aetna and receive any applicable allowance as specified in your plan documents.

*A contact lens fitting exam is not covered.
