Creditable Coverage Notices & Distribution Requirements

For groups enrolled in the Health Connections 2 (HC2) suite of plans.

All CBIA Health Connections 2 medical plans, with the exception of the following HSA-compatible plans, are creditable.

Non-Creditable Plans

• CIGNA $1500A Open Access Plus
• ConnectiCare POS $2500B
• Health Net PPO $2500C
• Oxford $2000D
• Oxford USA PPO $2000D
• All $2500 E Plans

CMS requires the distribution of a Creditable Coverage notice annually, no later than November 15. You should provide a copy of the HC2 Non-Creditable Coverage Notice to any employees enrolled in one of the non-creditable plans listed above. All other employees enrolled in a medical plan through HC2 (not listed above) should be provided with a copy of the HC2 Creditable Coverage Notice.
