Under Connecticut law, a producer can only act as an agent for an insurance carrier once the Connecticut Insurance Department approves the carrier’s Application for Appointment. Producers must hold an active Connecticut Health/Life license and have appointments with all CBIA Health Connections partners to receive commissions. Should commissions be due to the producer’s associated agency, the agency and producer must have an active Connecticut Health/Life license and the necessary appointments.

In addition to state licensing and appointment requirements, CBIA requires new agents to sell one new business case as part of the certification process on our products and services. Please complete and submit the following forms.
